Membership Form for Friends of the Library

The Friends of the Anne West Lindsey District Library (“Friends”) is a non‐profit organization that helps raise funds to support library programs and initiatives. Whether you are a new Friend or are renewing your membership, we appreciate your support! In addition to helping support your library, your donation also makes you a member of the Friends’ organization. As a member, you will be eligible for various benefits, which are based on the membership level you select.  

To join the Friends or renew your membership, simply complete a membership form with accompanying donation online or by paper – instructions for both are below. Thank you for your support!

If you would prefer to complete and submit a paper membership form, you may click here to print your own form or pick a form up at the library. Paper copies of the membership form and donations may be dropped off at the library or mailed to: Anne West Lindsey District Library, 600 N. Division St, Carterville, IL 62918. Please make checks payable to “Friends of the Anne West Lindsey District Library”.

The membership period is April 1 through March 31, regardless when you submit your membership form. Membership donations are not pro-rated.
